Improvements in the diagnosis, treatment and management of cardiovascular disease (CVD) mean that primary health care professionals are increasingly involved in caring for people with this condition. Although evidence based care is being delivered for patients once they have developed cardiovascular disease, little structure is in place for identifying and undertaking CVD risk assessment and supporting individuals found to be at high risk.
We now have the potential to delay or prevent cardiovascular disease developing through the use of evidence based guidelines. This module enables health professionals to develop a sound and comprehensive knowledge base and ability to undertake accurate cardiovascular disease risk assessment and apply that knowledge into their area of work. The module fosters a multidisciplinary team approach to care, taking into account the complexity of risk assessment and current service provision.Suitable for
The module is suitable for healthcare professionals working with individuals at risk of CVD in the community. This is likely to include Practice Nurses, GPs, community pharmacists and occupational health nurses. The course might well be useful for prison nurses, pharmaceutical representatives and health or personal trainers and other appropriate Health Care Professionals.

- Identification of individuals at risk of developing cardiovascular disease
- Undertaking risk assessment using risk predictor tools
- Non pharmacological & pharmacological management of individuals identified as being at high risk
- Communication of risk to individuals
- Motivational strategies to support individuals self care
- Organisational structures required for effective systems in your area of work
Learning Outcomes
By the end of the module it is expected that students will be able to:
- Evaluate the role of the healthcare professional in the detection and assessment of individuals at risk of cardiovascular disease
- Discuss the impact of cardiovascular risk assessment on healthcare resources
- Discuss the implications of CVD risk assessment for individuals
- Evaluate the impact of evidence based treatments in reducing co-morbidity and mortality
- Discuss strategies to encourage and motivate individuals to address lifestyle issues
- Demonstrate an understanding of how to apply the practical skills required for CVD risk assessment.
