Heart Failure: Diploma Level Module

Notional study time: 300 hours
Duration: 6 months

Improvements in the diagnosis, treatment and management of heart failure mean that primary healthcare professionals are increasingly involved in caring for people with this condition. We now have the potential to improve quality of life of people, increase life expectancy and reduce hospital admissions through the use of evidence based guidelines. This module enables health professionals to develop a sound and comprehensive knowledge base and ability to deliver holistic, evidence based care to people with heart failure, whilst also considering the needs of their carers. The module fosters a multidisciplinary team approach to care, taking into account the complexity of the condition and current service provision.

Suitable for

The module is suitable for Health Care Professionals working with, and caring for, patients with heart failure in the community. This is likely to include Practice Nurses, Community Matrons, District Nurses, GPs, Palliative Care Nurses and Cardiac Rehabilitation Nurses wishing to develop services in the community. The course is also open to Discharge Planners, Community Pharmacists and other appropriate healthcare professionals.

Learning Outcomes

By the end of the module, it is expected that students will be able to:

  • Analyse the impact of heart failure impacts on a patient's quality of life
  • Design holistic care plans for patients with heart failure
  • Implement evidence-based guidelines to optimise the drug treatment of heart failure
  • Evaluate how the provision of multi-disciplinary services across primary and secondary care influences patient centred services
  • Analyse appropriate health promotional activities to empower patients to manage their condition.
