The perfect introduction to managing long term conditions in general practice

General practice is often the first port of call for people with a health problem.  Along with acute illness that problem could be anything from poorly controlled asthma to needing education about diabetes; from wanting advice following a heart attack to needing help with managing the lifestyle implications of chronic heart failure.  All of these issues can and will be dealt with by the general practice team; this makes general practice one of the most rewarding and exciting places to work and the role of the practice nurse in managing long term conditions is invaluable.

This 5 day course looks at how to understand the key issues in managing chronic diseases such as asthma, cardiovascular disease, renal disease and chronic obstructive pulmonary disease; it also provides an overview of the role of the general practice team and the principles of managing long term conditions.

Aim of short course programme

This interactive five day short course programme provides an introduction to general practice for nurses who will be involved in managing long term conditions and meeting the requirements of the new General Medical Services contract through the Quality Outcomes Framework.

Short course programme objectives

Those attending the study days will gain basic knowledge and practical skills in the following areas:

Day 1 - Overview

  • What is general practice?
  • Working as part of the team
  • The primary/secondary care interface
  • How to run a clinic / call and recall systems / IT systems
  • Record keeping
  • Finding information and using it in practice - implementing the evidence
  • How to develop a protocol

Day 2 - Long Term Conditions

  • The principles of managing long term conditions, including the psychosocial aspects of:
  • Coping with chronic illness
  • Understanding the role of health promotion and health education
  • Lifestyle and behaviours as a risk factor for long term disease

Day 3 - Respiratory

  • Comparing and understanding asthma and COPD
  • Understanding the role of allergic disease and rhinitis in respiratory care
  • Inhaler device and nasal sprays practical session
  • Lung function tests

Day 4 - CVD

  • Anatomy and physiology
  • Prevention and management of CVD, including drug therapy and risk assessment
  • Understanding tests: the role of blood tests in managing long term conditions
  • The importance of accurate BP measurement and ECGs

Day 5 - Diabetes / Renal

  • An overview of the disease and its links to other long term conditions
  • Drug therapies used in diabetes
  • Targets in diabetes management
  • Renal disease
  • Understanding how renal disease is diagnosed, monitored and treated
  • The links to other long term conditions
  • The impact of renal disease on drug metabolism.

Duration and format

Five training days with informal lectures and interactive practical sessions.
Each day can also be accessed individually.
