This workshop is for nurses caring for patients with long term conditions

  • In England 15.4 million people have a long term condition
  • It is estimated from current demographic projections that by 2025 this will rise to 18 million
  • Managing long-term conditions is seen as the biggest challenge facing healthcare systems worldwide
  • People with a long term condition account for 80 per cent of all GP consultations
  • People with a long term condition are twice as likely to be admitted to hospital

This workshop is suitable for community matrons, community pharmacists experienced in medicine management, specialist nurses and social workers in the field of long term conditions.  It may have particular relevance to a local multidisciplinary team aiming to streamline care.

When individuals are coping with several long term conditions and in particular when their quality of life is significantly changed, they need co-ordinated support from professionals who will work with them to improve the situation. The current approach to managing long term conditions encourages a team approach between health and social care, including the patient and their carers in the team.

This workshop focuses on holistic case management, based on preparatory work from the students using the workbook provided. Drawing on local and national data, the group pools their expertise to develop creative solutions to problems that their patients have identified, and the skills of shared decision-making can be learnt and practiced in a safe environment with others who are working in the same field.

This workshop will give you the knowledge and skills to:

  • To identify and discuss complex problems in case management
  • To develop skills in shared decision-making
  • To be aware of new tools in problem solving
  • To encourage networking between case managers both locally and further afield

This is a one day workshop with informal presentations and interactive practical sessions led by a practising clinician. Students will be sent a workbook in advance, and the preparatory work is central to obtaining maximum benefit from the course.
