This course is for nurses who care for patients with long term conditions in practice or in the community
- In England 60% of adults report a chronic health problem, with 8.8 million people having a long term illness that severely limits their daily life. In the UK approximately:
- 8.5 million people have arthritis
- 3.7 million adults have asthma
- 2.7 million people have coronary heart disease
- 1.5 million children have asthma
- 1.3 million people have diagnosed diabetes mellitus
- 1 million people have diabetes but have not received diagnosis
- 0.6 million people have been diagnosed with COPD
By studying Education for Health’s Principles of Managing Long Term Conditions Module, you will gain the knowledge, skills and attitude to confidently support and manage people living with chronic disease.
This module covers the following areas:
- Definition and perceptions of chronic disease
- Issues for people living with chronic disease and for their carers
- Organisational issues, pathways of care and managing change
- Undertaking Case Management and holistic care
- Education for people living with a long term condition
- Evidence based practice
- Palliative care
- Effective consultation skills
- Supporting Self Care and teamwork
- Delivering effective disease management for long term conditions
- Professional Development
By the end of the module you will be able to:
- Examine the nature and perceptions of chronic disease management
- Analyse the principal issues for people living with and managing a chronic disease and those for their carers
- Critically evaluate the role and effect of organisational issues in chronic disease management
- Critically examine the principles of evidence-based medicine and practice in chronic disease management
- Assess and examine the principal issues for health professionals in managing people with chronic disease.
This course will take 6 months to complete and is delivered through comprehensive study materials supported by 2 study days.

This module is offered as part of the Diploma of Higher Education in Chronic Disease Management, which is a collaboration between NRTC (now Education for Health), Warwick Diabetes Care (at Warwick University) and Heartsave.
